Please allow 2-3 business days for a reply.

Do not use the website for same day appointments or sick visits. We see patients by appointment only for health checks, consultations, and sick visits. Saturday appointments are reserved for patients with acute illness. For sick visits, please call as early in the day as possible so that we can evaluate your child.

Reschedule an Appointment with PAL


Appointment Terms:
1) Insurance card must be presented at time of visit
2) If a person other than the child’s parent or guardian is brining the child to their appointment, a letter of “Permission to Treat” must be presented at time of visit
3) Please bring shot records to well check if this if your first visit
4) Co-payment is due at the time of service
